Outdoor cat has scratch across nose and ear, help

My cat really likes to prowl at night and I am worried that these scratches are either from a cat fight with another cat.... or worse a damn raccoon. I am worried about germs and viruses. Is there something I can do about this at home, or do I NEED to go to a vet?
